CANCELLATION CONTRACT

This Cancellation Contract (“Contract”) is entered into by and between:
Genoma Bioinformatics LLC (“Company”), located at [Genoma Bioinformatics], and

The parties agree as follows:

1. Cancellation Request

  1. Process: The Client must submit a written cancellation request to the Company by signing this Contract.
  2. Form: The cancellation request must include the following information:
    • Client’s name and contact information.
    • Subscription tier and account details.
    • Reason for cancellation (optional).

2. Effective Date of Cancellation

  1. Timing: The cancellation will take effect on the last day of the current billing cycle.
  2. Confirmation: The Company will send a confirmation of cancellation to the Client within 5 business days of receiving the signed Contract.

3. Data Handling After Cancellation

  1. Data Retention: The Company will retain the Client’s data for up to 30 days after the effective date of cancellation to allow the Client to retrieve it.
  2. Data Deletion: After the 30-day retention period, the Client’s data will be permanently deleted from the Company’s systems.

4. Refunds or Credits

  1. No Refunds: No refunds or credits will be issued for unused portions of the subscription.
  2. Outstanding Payments: The Client must pay any outstanding fees or charges before the cancellation is processed.

5. Additional Terms

  1. Notice Period: The Client must provide at least 10 business days’ notice before the desired cancellation date.
  2. Penalties: There are no penalties for cancellation, provided all outstanding fees are paid.

6. Governing Law

This Contract is governed by the laws of the State of Nebraska, without regard to its conflict of law principles.

7. Entire Agreement

This Contract constitutes the entire agreement between the parties regarding the cancellation of the subscription and supersedes all prior agreements or understandings.
